Mystery APP “Feedback” in iPhone and MacBook Air

Hi Friends,

Unknown feedback app appeared in my IPhone and MacBook under Accessibility/ per app settings/ Feedback.

it is not found in app list, cannot be deleted or activated.

Does anyone has idea what is it?
