If you can’t change your region because of a balance, you can spend your balance on purchases, even if it exceeds your account balance. For the extra amount, you will pay using another payment method. All purchases will remain even after a region change.
If your balance is less than the cost of a single item, you will need to contact Apple using Get Support to empty your balance: https://getsupport.apple.com/?caller=kbase&PGF=PGF63005&category_id=SC0998&symptom_id=20198
If you’re trying to get a refund, unfortunately, gift cards and balance in your Apple ID is non-refundable, except as required by law. If your jurisdiction allows gift card refunds, contact Apple Support: Get Support: iTunes Store account billing
