I have elderly parents who have a single iPhone. We like the fall detection on the Apple Watch and it has come in handy when my father fell. My mother is now also getting unsteady and we'd like to be able to give her an Apple Watch to detect any falls she may have, but they only need a single iPhone between them. Is this possible?
On their one iPhone, they could set up the 2nd Apple Watch as a family member's Apple Watch: Set up Apple Watch for a family member - Apple Support. The fall detection is operative on a family member's Apple Watch.
The only requirement of this setup is that the Apple Watch must be a cellular model. You don't need to have a cellular plan for the watch, though.

Yeah, that's what I've read. We don't care about the health tracking--just the fall detection. My guess is that you can have multiple watches synced with a single phone, but only one watch will be active at any one time. Mean, they can both be synced, but only one of the watches will detect falls (whichever the phone considers the active phone). Thanks for taking time to answer!
