I'm not following you on this question? If the charge is lower than 100% and the phone is plugged in to charge, the lightning bolt will appear whether you are using the phone or not, if that helps. But once the phone reaches 100%, the bolt goes away.

I understand what you’re saying.
While plugged in, iOS used to show a plug graphic inside the battery icon to indicate that the battery is fully charged. Right when it went from 99 to 100, there still was a bit more charging to do so it would show a lightning bolt until it was fully charged, then it would change to a plug to indicate it was on external power entirely, and that the battery wasn’t being charged anymore.
They removed this a few years ago. I miss it too, it was nice for those with OCD who wanted a fully charged phone.
Because it is no longer charging, so the lightning bolt goes away. The phone charging system is built into the phone. The charger only supplies power to charge the phone.

Yes, you are being repetitive, but that's OK. I already explained why. There is NOTHING wrong with your phone and it is doing exactly what it was designed to do.

When it was charging the lightning bolt symbol was there, but once it became 100% charged it goes away. Why is that?
