Once an app has been downloaded (either previously approved through Ask to Buy or through other means), the permission can't be revoked. You can, however, block all app downloads from the Store which should fix it. Delete the app, then go to Settings > Screen Time > Content & Privacy Restrictions > [toggle on] > iTunes & App Store Purchases > Installing Apps, then choose 'Don't Allow' under 'Store Purchases and Redownloads'.

That would work. Did you try asking Apple Support about it, though? I fail to see any commercial sense behind Apple actively undermining their own system of parental controls just to 'outsmart parents'. After all, it's typically adults purchasing these devices, not children.

Even after disabling everything under 'Redownloads'? I'd contact Apple Support then:
You could potentially give the app a limit of 1 minute, too- while not technically blocking it, it's better than nothing.
