Do not disturb turns itself on and will not turn off

I am getting told incoming calls and messages are blocked. Do not disturb seems to be on all the time. I set a schedule, but it does not turn off. Have have a sleep focus, but Do Not Disturb does not turn off I have turned the phone on and off multiple times. Sometimes I can delete the Do Not Disturb schedule and get it to turn off, but then later it turns on again and won't shut off.

When I go into the Control Center and look at the Do Not Disturb, there is NO option to turn it off!!

The iOS is the latest update.

Is there damage to the iOS? Is this a bug? Is this a phone malfunction? This is a rather new problem.

How do I troubleshoot this?
