Review apps that are allowed to save photos at Settings > Privacy & Security > Photos. Once permission has been granted, they can save any picture to your Photos library.

Review if anyone else is signed into your account at Settings > [name on top], and scroll to the bottom to see if there are any other devices that are logged in and can save a photo to your library if iCloud Photos is on.

Review Safety Check at Settings > Privacy & Security > Safety Check, to see if there is any information being shared.
