Ultimately, the solution seems to be wait or fully reset your device and don’t do a restore. I’m more than happy to run through all the troubleshooting steps including some that haven’t been mentioned above. You can run through the troubleshooting with Apple Support and they can also do remote diagnostics on the remote possibility it’s a hardware issue. I doubt hardware though because you received the Terms and Conditions.

Yes, and the bank can tell you if the tokens assigned to the card is approaching the limit. Banks limit the number of tokens that can be associated with the account/card. So, adding the card to a number of devices can cause issues.
Just let it sit a couple of days and try it once.

Thanks very much for your help. I have just tried added my card on my wife’s phone and it worked straight away so it surely has to be something wrong with my Apple wallet doesn’t it? The card is clearly not blocked as worked on my wife’s phone. Iv rang the bank and spoke to Apple on the phone twice but they still can’t work it out
No, that’s very common. Everything is device specific. The multiple attempts is probably still affecting your device.
There are several unique identifiers for your device that are shared with both the issuing bank and payment network operator.
It says ‘card not added’ contact card issuer. Which I have done but they can’t find anything wrong

I have just tried adding the card on my wife’s apple wallet and it worked so it can’t be the bank/card that’s the problem it has to be something with apple wallet doesn’t it?
