Logged in incorrectly 1 time, and got a 59 minute lockout?

Any idea how this happens? I logged in one time, I swear my PIN was correct, but then it just spun and spun, then suddenly gave me a 59 minute lockout. This was not after multiple invalid logins, but just one attempt to log in. It's happened multiple times, I also use a security key.

MacBook Pro 16″, macOS 14.4
