the accessibility highlight turns on at random

I have a brand new iPad Pro, it's about 3 days old. I keep having the accessibility features turned on at random intervals. I hardly turn the device off, so I am pretty sure I have not hit the power button 3 times in a row to turn on the feature.

I repeatedly check the accessibility settings only to find everything in the same state I left it (off). The only way I can remove the accessibility feature was to toggle the guided Access feature on and off (It's default state was always OFF!!!)
