I'm not sure what device you are using, but the way to set up events to repeat every 3 months in iCloud calendar is similar.

First, open the Calendar app on your iPhone, iPad or Mac.

Then, create a new event you like. Different devices may have slightly different steps for this operation. You can click on the “+” button on iOS, or double-click the date on macOS to create a new event.

Next, fill in the details of your event such as title, location and time. Then, please find Repeat, and choose Custom. You can then set the event to repeat every 3 months.

Below is a screenshot regarding the Repeat settings on iPad for your reference.

After setting the event to repeat every 3 months, make sure to save the event.
