Delete the Teams App from your iPad - and then perform a forced restart. This will reset the App permissions:
Remove apps from iPad - Apple Support
Force restart iPad - Apple Support
After successfully restarting the iPad, reinstall the Microsoft Teams App from the App Store:

Once re-installed, launch the App - allowing access to Camera and Microphone when prompted.
