When spacebar is pressed on file, it is outlined in blue (focussed). All keyboard focus is turned off. Cannot find reference to this in accessibility options. Where else to look? There is no combination keyboard/mouse setting referenced in other solutions.

It could be due to the Full Keyboard Access functionality. On your Mac, go to System Preferences, click Accessibility, click Keyboard, click Navigation then disable Full Keyboard Access.

This also resolved the vexing issue of quicklook not working.
Note to others: This option is found specifically under Accessibility, then keyboard, as opposed to the
many settings under keyboard/shortcuts/keyboard.
