how much does it cost to upload an app to app store?
I want to upload an app, but everywhere, it says they charge $99 per year!!!! the app would be free without in app purchases. also, what kind of file type would the app have to be?

For iPhone and iPad, USD$99 per year is the cost of admission to the app store, yes.
Xcode deals with the app packaging and the ipa file extension and other such details for you.
If you're using third-party development tooling, check with whoever is maintaining that for tool-specific details.
