Where exactly are you looking.

In the Contacts App or the Contacts tab in Phone App the + icon is at the top right corner.

If you are in another App, then it may or may not have a way to add a contact.

Creating New Contact from an Incoming Phone Call Ever since IOS was updated to v26x I can't create a new contact after receiving a p[hone call that shows the caller's ID info (ie first and last names). In stead of being auto populated, I have to write down their name so I can add them to my contact list.
