In Preview, in the drop-down menu for line width, there are some options for variable width, where a line stars out thin and gets thick. But I can't figure out how to implement these. They seem to have no effect on the lines I draw.

Yes, Mac Pro & Sonoma - dialabrain has the correct answer - In pen drawing mode I choose one of the options in my screen shot of varying line width (line starts thin, gets thick) and then use finger pressure to activate the feature.
Is this true and accurate " MacBook Pro 14″, macOS 14.5 - Sonoma ?
If yes, the available options using Preview version 11.0 in Sonoma 14.5 are as per below Screen Shot
What is presented in your earlier image, does not appear to available in this version of macOS
You click on the desired line size in that markdown menu before you attempt to draw your line segment.
