Mac Mail on my iMac desktop now only shows the primary email address associated with my account. I can't select my other apple email addresses to send mail from. The additional email addresses continue to be available on my iPhone. How do I get these additional email addresses back on my desktop iMac?

Safe Boot, (holding Shift key down at startup), does the problem occur in Safe Mode? Could take 10 minutes or more.
Safe mode attempts to repair Disks & clears lots of caches & loads safe Drivers, & prevents loading of 3rd party extensions, so if Safe Mode works try again in regular boot.
Perchance was you missing email address through Cox Cable? Those were moved to Yahoo and lots of people are complaining about the transition:

Thank you. I tried doing a safe boot and the problem exists in safe mode as well, and continues after I reboot normally.
