My Mom had the parental control on my phone and now on my contact card on my phone it keeps popping up as my mom’s number (323) and not my number (562). Whenever I change my mom’s contact name as mom, on MY contact card it says mom and not my name. Help!!!

Set this Up as your OWN iPhone:
You need to have your mom remove your iPhone from your mom's Apple Account. Then, you need to create your own Apple ID. From there, sign in to this with your own Apple ID. Note that Apple IDs should never be shared; you show have your own Apple ID.
