The problem seems to exits for a long time and Apple is doing NOTHING about it. See here Email photo as attachment, not embedded, … - Apple Community. This is very annoing. The comment about Outlook seemly being the only problem is not true. I just tried it with gmail and got the same - bad - result. Apple, fix this!

If only that were true. Unfortunately, it isn't. The image is embedded in the email. It makes the download of multiple images much harder. Instead of 10 seconds, it requires minutes with multiple images. A significant reduction in efficiency. Updates should improve things (back in the day images were handles correctly) and not make things worse.
There remains nothing to fix. Photos and files are attached, the email client is displaying the image. You can still download the attachment.
