Location is off. Find my iPhone is off. Disabled share my on screen time. Often find my iPhone shows up on my app report. Today I took a screenshot of this. But when I went back it was toggled off. This happens randomly as well. Concerned that my personal phone is being controlled or randomly hacked by an ex with expertise as an IT. Please help resolve so this doesn’t continue on.

It's extremely unlikely that your iPhone has been hacked. That's very difficult even with the sort of tools people outside of government agencies cannot get their hands on. If Find My is being turned off on your iPhone, most likely your ex or someone else has your Apple ID and password. Log into your Apple ID account and change your password. Then see if it continues to toggle off. If it does, post back.

An app privacy report is what is being shared to developers of specific apps. There is no way an ex would have access to those reports.
I’ve done that and more. Resettled my phone back to manufactured settings, created a new iCloud acct with a new password. Find my is off as I have it, that is not changing. What I am concerned about that (share my) toggles on and off by itself, also find my shows up on app privacy report when I don’t use or go into find my app.
Not sure what App Report you are concerned about. Do you have a screenshot of this repost? It appears you do have Find My and Location Services shut off, is that what you expect?
