I hadn't used it before, but if you point the iPad camera at a document, it detects it and goes into a scan mode. Then if gives the option to save to Files, not to Photos. It is saved as a pdf.

What Photo app exactly?
The default Apple Photos app does not have a document scanning feature.
If using the Apple Notes app, the scanned document is saved in the Note that was opened when the scan was started. Nowhere else.
If you are using a different App, please identify it clearly.
Did you use Notes to do the Scan? Then the result goes in Notes. Or what did you do?

Oh, if you use the Camera app, then you can save the result to Files. Find the Files app and look there.
Other apps may do something different...

Does not seem to work on my 9.7" iPad Pro running iOS 16.7.10 or my iPhone 15 Pro Max running iOS 17.6.1
No scanning option appears at all. I get a Translate option and copy option on my iPhone 15 Pro Max though.
Maybe only in newer Apple silicon iPads?
I can scan from my iPad through the Photos app if I use the continuity camera option form my Mac though.
Looks like this:
A barely visible yellow outline appears, indicating text, I think, and the little yellow "Scan Document" button appears -- you tap that to take the scan, and it zeros in on the receipt, a blue haze appears over the area to be scanned, and you hear a click. Fascinating!

It seems prudent not to show the whole thing....
Mine is an iPad Pro 11 inch 3rd Generation with iPadOS 17.6.1. It doesn't happen on my iPhone 12 Pro.
The little pill shaped "Scan Document" badge does not appear for me on the iPad.
Might be an iOS 17 thing on iPad only though.
