Your Apple ID is something only you would know. There is know way to reset it if your forget. Apple does offer tips to help you find it:

Once you find your Apple ID, the Password can be reset by going here:

As for finding your lost device without your Apple ID and Password, it will not be possible, unless you have another device signed into your account or are sharing your location with anyone else.
