I have an old but still useful Iphone that was restricted because it was temporarily lost and we were concerned about fraud. But in the meantime lost the apple id/password combination. Can't seem to fix this online - does that mean we should go into the apple store & physically confirm identity & ownership of the phone? Seems like a circular problem because we need Id to get onto internet to get into apple.com to fix id.......

You must be referring to the Activation Lock. The only ways to unlock your iPhone is with the original Apple ID login or provide Apple with the proof of purchase receipt.
