Is it possible to sync the passwords in the Passwords App on the Mac (Sequoia)
with the Passwords App on the iPhone (iPhone 15 Pro / iOS18) WITHOUT using iCloud?

Sorry, no. I just did a test: the password created on my iPhone (iOS 18, iCloud Passwords turned off) was not synced to my Mac (macOS Sequoia) after doing a Finder synchronization.
This being said, Apple protects very well the iCloud Keychain:

Apple passwords is just a GUI for iCloud Keychain it syncs across all apple devices and has done for years so yes it will work
