When playing games my gpu usage will be around 80% and just drop to 20% randomly causing the game to then lag, I was wondering if anyone had any similar issues and where able to fix this?

It sounds like the Integrated GPU in your 21.5" iMac is overheating and throttling down to protect itself. Plus repeated overclocking and overheating from gaming, will cause GPU to fail sooner than later.
Having the iMac cleaned and using https://crystalidea.com/macs-fan-control might help, but ideally you should be using a Gaming PC with a Discreet GPU and better ventilation for gaming.
