Passwords App Key doesn't show in Sequoia menu bar

I am trying to add the Passwords App to the menu bar. In Passwords app I've gone into Setting and enabled Show Passwords in Menu Bar; restarted my Mac and it doesn't show up. Thoughts?

System Setting > General > Login Items & Extension > Enable "Passwords" toggle in Allow in the Background.
Open Passwords > Go to "Passwords settings" > Check "Show Passwords in Menu Bar"
