I’m ordering a 46mm watch and it’s asking if I want a small- medium or medium to large band. How would i know. Is it based on the size of the watch or the size of my wrist? If wrist, what sizes are covered by each?

Well, that is unlikely. If it is 160 mm (16cm), it would probably be considered a Large, as per the screenshot below.

If you're near an Apple Store, you can try them on there.
It's based on the size of your wrist.
If you have a large wrist you'll want a large band.
If you have a small wrist, you'll want a smaller band.
Measure your wrist, and convert that to millimeters. Then you can choose a band size for it.
Your wrist cannot be 16mm. That would be positively tiny. Like a twig, like your pinky finger.
Maybe 16cm which is 160mm. Yes, that would be small /medium. As seen in the Apple Watch band sizing:
