Apple Watch Fall Detection

My Apple Watch series 9, running is 10.6.1 does not fall detect. This is the primary reason I overspent on this watch. Twice now, full fall down accidents, and no fall notification at all. How do I fix this? I have reset watch and phone, turned fall detect off and back on, restarted them too and nothing.
