Apple’s supplied Malayalam font is defective. Ask them to fix it via

Noto Sans Malayalam does it correctly, but this is not available on the iPhone.

PS with an app like AnyFont you can add Noto sans Malayalam to your iOS device and use it in apps which have a font menu, like Pages.
