Why do my wired headphones disconnect on iOS 18 and 18.1?

When playing music using wired headphones (usb-c) headphones constantly disconnect not physically but music randomly decides to play aloud/skip entire queue. Replaced headphones and is a new iPhone 16pro same issue & so is obviously software related.

any wired headphones users experiencing the same problem?

Also 1/2 the time the in-wire control doesn’t control anything until the wired headphones are unplugged and replugged in again.
