Unable to send mail. Outgoing Mail Account: iCloud (Offline)

This problem keeps returning at least once a year. Emails get stuck in the outbox. The "connection doctor" can connect to IMAP but not SMTP. In the past the only thing that seemed to work was to completely remove the iCloud mail account from the Mac and rebuild it. Any smarter suggestions? Any chance Apple are going to fix this ever?
