missing music on iphone

Completely random songs are going missing my wife’s iPhone.

like a entire album was added and downloaded. Now only 1 song is left on phone. In both library and downloads.

songs are still available on Apple Music but not added anymore on the phone.

as per previous questions here optimise storage is off and has 94gb free on phone.

sync library is on.
