Parental control fails sometimes

My children's browsers are set to only show approved websites and it usually works. But sometimes it suddenly stops blocking sites that are not allowed, but all the settings in all the phones still seem to be correct and the software is up to date.. I have changed the codes but it still doesn't work. Does anyone have a clue what it can be?
