Am no expert in this area, yet your post to the community deserves some response, although I may be very much in the dark about your particular needs.
Seems to me the screen-sensitivity may be a matter of fine tuning the settings on the iPhone 15 Pro Max.
You may or may not need a sighted person to help check, change, or customise the settings to suit.
As your iPhone 12 seems set-up with settings that work for you, checking the new phone for similar settings may be helpful, yet the iOS versions and app features may or may not differ.
Also the iOS 18 installed on the iPhone 15 Pro Max may need updating to the newest iOS 18.2 version.
You may want to go to Settings > General > Software updates to check. Note: If you want to update the iPhone, it is wise to back up the data first.

Wondering which settings you have in place, and how they differ from the settings you are used to. Also: As new accessibility features become available on iPhone, sometimes it takes time to learn to benefit from their usabiliity. You may want to adjust settings for taps, swipes, and multiple touches, using the linked article below:

Seems there is a setting in place that that triggers that result.
We do not know if you are using VoiceOver to control the screen, or are just using "Speak Screen".
As only one character is read ... seems you may be using the Speak on Touch settings (where you tap the text you want iPhone to read).
In Settings > Accessibility > Spoken Content you can enable either:
- Speak selection (eg: select a phrase, sentence, paragraph)
or
- Speak Screen (ie: to have all text on the screen read aloud beginning from the top of the page)
You may require different settings for different apps ... eg: reading all text on the screen for the Books app
