There are loads of reasons why a photo may came out blurry or grainy. Did you hold the camera steady? Were you using the right camera for the scene you were trying to take a photo of? Was the light too low and you didn't hold the camera long enough for night mode to work correctly? Do ALL photos you take come out blurry or grainy?
Without knowing more about the photo, it would be difficult to help you. And asking only other users to fix what they couldn't possibly know more about won't get you anything. Maybe start by posting the photo you took.

There may be two issues at play here. One is it was shot in Portrait mode. Please note the f/number symbol in right corner of information. It’s to the right of HEIF. The f/number can be adjusted in the Photos app.
However, I don’t think that is the issue. My suspicion is you exceeded the minimum focus distance that all lenses have.
All lenses can focus to infinity, think way out in space. But all lenses are restricted in how close they can focus. I just tried reproducing a shot with a face that close, at 1X, in portrait mode and I couldn’t get that close and keep my subject sharp.
Try taking a step backwards so the lens can focus within its range, or using the 3X or 5X lens.

I agree with Jeff. You appear to be too close to the subject and the camera is not capable of focusing correctly in Portrait mode. But you said you stepped back. Take the same photo more than a step back. Take the photo from a few more feet back and then post it.
We are just fellow Apple users, we can't fix hardware issues, but we can try to assist. Start off by making sure your iPhone is up-to-date, if it is, then do a restart on the device.
Please share a couple of images so we can see exactly what you’re seeing and try to properly diagnose the issue.
I tried everything lightning’s it was steady also my husband has the same 15 pro max but he doesn’t have any issue with it but in my phone its too grainy
Look its too much grainy in the eyes and the mouth area too no more clarity
Thank you for sharing. Which camera/lens did you use, 1X, 5X or what zoom setting? Thanks.
There was no zoom just 1x even though i tried with all 2x and 5x its same too much grainy and blurry
Im not clicking my pictures out of space and i tried taking a step backwards also its the same
Yes, I see the issue you’re referring to. Just a few more questions, please. What is the ISO and shutter speed the photo was recorded at? This information is available when you open the image in Photos app. Just tap the > I < button in the circle at the bottom of the screen.
