Screen Time using "Last Name" instead of "First Name"

My son's Apple ID is set up with his first and last name. When managing his screen time, he is referenced by his last name instead of first, (Managing "Last Name's" Screen Time). His name appears correct under the "Family" section in screen time but his circle with the initials is showing last / first. Everywhere else it shows him set up with first name / last name, even under where you manage the family members. My other son's account has no issues and refers to him by first name.
