My notes icon has disappeared from my Home Screen ?

The app is probably still on your phone, but just hidden or on another Home screen. Try this:

Swipe down from the middle of your Home screen to open up the search screen. Then enter Notes.

If it doesn't appear, download a copy from the App Store.
