You can't delete the following Apps from the iPhone
Photos, Camera, Clock, App Store, Health, Wallet, Safari, Phone, Message & Find My apps.
Check for it under the App Library. Next to the last page of the Home Screen. If it is in the App Library Touch, Hold and Drag the icon to Home Screen. That's it.

A long shot, but have you tried resetting the Home Screen layout?
Settings -> General -> Transfer or Reset iPhone -> Reset -> Reset Home Screen Layout
Yes, at this point I'd suggest reinstalling iOS.

Thanks for the quick reply. I had already tried that - it's completely missing from the phone. The home screen camera icon also doesn't work. I can't access it from iMessage either. It's not accessible within settings either. Happened immediately after updating.
That didn't work - it seems like a bug or something went wrong during the update..
I really don't want to have to do that. I feel like the authentication apps always get screwed up. I'm hoping Apple pushes a new update
