What is the exact description of the charge?

You have authorized a subscription, even though this may have happened inadvertently. How you proceed will depend on the description of the charge to identify the source. If the source is Apple, then you can cancel the subscription here:

Also, if you do not see the charge in your Purchase History, it is not from your Apple ID. If you do see it there, then changing your password may be necessary.

It is your bank that is updating the merchant with your new card number. It is a service they provide called Automatic Billing Updater. The scammer has not hacked your account/device to gain your account number, your bank is giving it to them because subscriptions are a contractual obligation to pay for the service until cancelled.
