Call Forwarding Symbol with No Number on iPhone

Call forward symbol appeared without any forwarding phone number and will not clear when turning off and on both setting and phone itself on restart. Last thing I find is to contact my phone carrier, I’m doing that next.

Should I be concerned that I am being hacked or tracked by either good guys or bad guys. Also how do I check to verify this is a non harmful anomaly? Is anything random in cyberspace? No!
