Loud alarm reminder before calendar appointments

You might try an app like Calarm. It links to your calendar and can play louder alerts. I haven't used it in a while, though.

You can also change the alert for Calendar in Settings>Sounds & Haptics>Calendar. You might want to poke around the iTunes Store and see if there are alert tones that you can hear better. I've got one that's an army bugle call that's quite loud.
