Re adding Ontario COVID immunization record to Health. Keep getting the message 'Unable to Add Data: This data could not be added to Health.'
On Ontario vaccination portal they say QR code “may” not work with third party wallet.

Unfortunately, the ability to add ANY card to Wallet is wholly dependent on the card’s web app developer.
You’ll need to contact the Ontario Health entity to resolve.
In the interim, simply scanning-in the card to Notes might provide you w/ a workable alternative.
If the health agency that issued the record does not support adding to Apple Wallet, it can’t be added.
Apple Health app I’m not familiar with. But again, when I added my immunization record, it was done using a link at the issuing authorities website.
