If two-factor authentication is not already enabled, enable it. That gets rid of the security questions.
If two-factor authentication is enabled, then (after two weeks) you have no security questions.
If you do not have two-factor authentication enabled, and if you have lost control of your Apple Account, then you’ll usually need to relocate to another Apple Account. Folks without two-factor pretty commonly lose control, and the adversaries are efficient at making what was once your account into their account.
