The phone is probably still locked to T-Mobile. You will need to contact T-Mobile to ask them to unlock the phone.
Apple cannot do this.
The fact that a phone was purchased at the Apple Store does not necessarily mean that the phone is unlocked.
Who is your current carrier?
Have you contacted them to ask about the lock?
Apple cannot unlock a phone that has been locked by the carrier.
Did you purchase the phone at the Apple Store using installments with AT&T?
When you purchase an iPhone at Apple, you’ll receive an unlocked model that you can use with any carrier. The only exception is if you purchase an iPhone through AT&T Installment Plans — you will receive a locked model.

For what it's worth, on iPhone ordering pages, words like "And your new iPhone will be unlocked" disappear whenever you select any carrier financing plan – not just AT&T's.
