The case is still in the courts. It will probably be 7-8 months or more before the settlement terms are known and claim forms will be available.
Check back here in 7-8 months.

The deadline has passed. There is no money for you. If you were eligible, and blew the deadlines, you have also given up any right to sue Apple yourself.
