someone is hacking my phone

My ex was able to send me a screenshot of my current call list from as well as recordings of my phone conversations. The screenshot looked like it came from an android or non apple device. He does not have direct access to my phone to install anything. How can I prevent him from using 3rd party apps or tools to see/hear/record my calls and seeing my recent call list? (The calls showed up by the name they were saved as in my phone, like mirroring, not the actual number or government name, but exactly how I have them saved in my phone)

No one can remotely hack your iPhone. The only way someone can hack your iPhone is physically, and that is called jail breaking, which is very difficult to do and requires lots of expertise and money. Unless your ex was willing to pay a few hundred thousand dollars to hack you, you are not hacked. It’s very possible that as muguy stated your Apple Account could be compromised, in which you should remove any unknown devices on it and change your passcode immediately.
