Are replacement phone brand new and never used ?

It’s unclear what it means when my replacement model number starts with an N. As a hold was put on my cc for the full new retail price and the original one sold he was defective I would think it would have to be new but it didn’t include the charger and others accessories included in a new purchase order. Is there a documented answer to this question from Apple?

Replacement phones are generally not new. They may be refurbished returns. Or, they may actually be "new" in the sense parts were taken from new phones that did not pass inspection tests and the various working parts were used to make a working phone. These are set aside as replacement stock.

Either way, all such replacement phones are essentially the same as new. They carry the same warranty as the phone you first had.

Starting with the 2020 iPhone 12 lineup, Apple iPhones no longer come with a charger or headphones in the box.

New unused iPhones are shipped in a shrink-wrapped box, and a protective plastic sheet cover over the screen.

Replacement phones may be in other conditions, depending on circumstances for replacing it. Refurbished models would start with an ‘R’.

Actually, brand new phones haven't come in shrink wrapped boxes for a while now. While they aren't shrink wrapped, they have a pull tab underneath the box on both sides of the box, the user pulls which releases the top box covering the phone. And the cover over a new iPhone is no longer a plastic sleeve, but is made of recycled paper and is an off white color.
