iPad not saving PDF changes

I've been using my iPad to review course content for a class. I highlight and write notes on the PDF, usually the "saving" notice pops up and saves my changes but there are a few times in which it will say that its saving and all of a sudden go to the 1st page of the PDF, that's when I know it didn't save anything I did. I tried hitting "Done" on the PDF and see if that helps but its hit or miss. This is really frustrating because i have to go back and hope I remember the key points that I had highlights/left notes on. I have iPadOS 18.3.1 on a iPad Pro (6th Gen)

Its a PDF I downloaded from the course website. I saved it as an open PDF in my iCloud after I downloaded it. It had been working just fine until I hit page 254... after that it won't save anything. I have downloaded an app that I can use in the meantime but its a trial period. I have always just edited notes on the PDF on my iPad.

If this PDF came from your instructor, the solution will depend on how they saved it. Can you do Save As and save your notes in the file that way?
