How to I restore my MacBook to a previous date so I can recover a pdf I was filling out? I was filling out a form and it reverted to the empty form.

If you were working on it in Preview, try checking File > Revert To > Browse All Versions to see if an older version is available. If the file was stored in iCloud Drive, you might find an earlier version in iCloud.com under the Recently Deleted section.
Otherwise, if you have Time Machine backups, you can restore the file from there. If none of these work, data recovery software like Stellar Data Recovery for Mac might help retrieve lost or unsaved files
