Adding the Flashlight Icon Back:

Not sure what you are looking to achieve here, but if you want to add the Flashlight to the Control Center, perform the following...

1. Go to: Settings
2. Tap: Control Center
3. Tap: Flashlight in the list of available controls.

On the lock screen press hard and then tap Customize. Since you've removed the flashlight tap the plus (+) sign that appears after removing the control to pull up the Control menu and choose the flashlight to put it back on your lockscreen.
